Answers Specimens have been given non- consecutive serial numbers. In some cases, the type of specimen has not been recorded. The laboratory serial number is reset to 1 each day. Sex and age not recorded for some patients. 5

Answers Transport of some specimens from the Uadouga health centre was delayed. Some dates of specimen inoculation have not been recorded. Most instances of incomplete information concern patients from the same centre. 6

Answers For one follow-up specimen, the months of treatment are not indicated. Some specimens lack the “reason for examination” information. After a highly positive sample, two low and prolonged-incubation positive cultures have been registered: possible cross- contamination? 8

Answers One 6-month-follow-up positive culture has not been sent for DST, but no comments or reasons have been recorded. In some cases, the name of the person reporting the results (and the signature) is missing. One negative culture has been reported before the 8-week deadline. 9
