Moving on from hospital living in Dorset – lessons learned 1

2 People who have now moved into their new homes so far: 1. Dorset: 43 people (80%) Where are we now? 2. Poole: 21 people (75%) 3. Bournemouth:28 people (53%)

3 People who are still waiting to move: April: 7 people May: 1 person June:1 person Who’s left? These delays are because some housing has not been ready on time and some people’s support teams have not had their criminal record checks or NHS pensions paperwork through yet.

4 Sods law “Anything that can go wrong will go wrong” What have we learned?

5 Things have gone well for most people who have moved but there are still problems Some people have already had to change their support provider Only one person so far has had to change their home (just for a short while) What have we learned?

6 Making person centred plans with people has been good but we need to keep checking that these are kept up to date after the move Some providers need to get better at offering day activities People now need to be supported to link in with their communities more to make friends and try new things What have we learned?

7 We need better ways of looking at people’s outcomes and measuring them against goals We will be asking providers to send us copies of everyone’s support plan so we can check what people are getting We will be changing the contracts with all of Dorset’s support providers so they have to deliver person centred outcomes Quality Checkers – into action! What needs to happen next?
