2. 2015 Pain Medicine Website Usage - D ata Collection Data Collection Data were gathered via mail questionnaires between February 2015 and May 2014. The sample received an original mailing in February and a follow-up in April. A monetary incentive accompanied each questionnaire. Two rotations of each questionnaire were distributed on an every-other-name basis. The websites were assigned a unique rotation on each version of the questionnaire.
