Kiewit. Driven by projects. Built by YOU. Contributing Factors Pulled on come along until we took it beyond its rated capacity. Nylon came from another project but appeared to be in good condition. Employee inspected prior to use. Nylon was wrong application for the task (intent was to use nylon to protect coating on inside of pipe). Configuration with nylon sitting in come along hook caused cut (nylon obviously cut not overloaded). Configuration and location of come along put employees in poor body position and line of fire. Crew had been struggling with first joint fit up for extended period of time and were not gaining on the joint (bound up). Crew did not regroup with superintendent and look at other options.

Kiewit. Driven by projects. Built by YOU. Never Never use broken or damaged come alongs or rigging. Never rig hook to load or chain. Never side load the hook. Never use a sprung hook. Never use a cheater bar. Never drag the load without knowing the weight.

Kiewit. Driven by projects. Built by YOU. 14 Always Always inspect come along and rigging prior to use. Always make sure load is seated on the hook. Always know the weight of the load. Always stay out of the line of fire where feasible.

Kiewit. Driven by projects. Built by YOU. Going Forward Best Practices Utilize steel chokers rather than nylon. We will explore all other options before using nylons. Steel chokers have less potential to break. Softeners and shackles can be utilized. Come along will be part of our tool training including emphasis on line of fire. No more than one arm strength use on come alongs as you can overload the system. Where possible utilize materials to absorb potential release of energy.
