1. Transportation Incident August 2013 Driver Injury – Struck by Falling Scrap

2. Some Facts Driver arrived at Omni Yard to unload container. Employee at Ferrous receiving area directed the driver to back into an open area to dump his material (relatively clear area). Driver decided to back into an open spot adjacent to this area (less clear).

3. Driver uses Boom Dog to open the container door keeping himself a safe distance from material that could fall out when initially opening it up. Material Handler operating too close to driver. Driver should be at least 50 feet from the operating radius of the equipment. Driver Must Communicate with the Operator of the Material Handler. Driver is not wearing his safety glasses. 7. How Could This Have Been Prevented? Driver responsible for being aware of his surroundings. Hazards seemed obvious in the pictures – Trip Hazards – Mobile Equipment Operating Too Close – Overhead Hazards – Potential Hazards Still Inside Back of Container

8. How Could This Have Been Prevented? What about other Employees in the Area? Does the employee taking the photos see these hazards? Does the Material Handler Operator see these hazards? What would YOU do if you see a Hazard or an Employee at risk of being injured?
