When is the IST to be Notified Any fire/explosion at AGLR office or property Any gas related fire/explosion within our area of operations Suspected business-related employee, contractor or visitor death Suspected business-related hospitalization of employee, contractor or visitor Incidents requiring notification of the National Response Center or OSHA Incidents expected to impact more than 100 customers Any need to request or provide Mutual Aide with another company Critical IT system outage estimated to be greater than four hours Building evacuation or quarantine of an AGLR office or property resulting from an actual incident or threat Unlawful occupation or disruption of an AGLR office or property

Notification Criteria Continued… Significant environmental contamination or toxic release Circumstance expected to generate significant negative press coverage Credible bomb threat Damage to AGLR facilities or injury to AGLR employees resulting from severe weather or other events An actual or threatened kidnapping of an AGLR employee, a hostage situation involving an AGLR employee, or a significant incident of actual or threatened violence against an AGLR employee Arrest of an AGLR employee for allegations of on-the-job criminal activity Notification of legal action with exposure that has the potential of significant business impact or negative publicity Notification to regulatory authorities of high profile incidents Death, severe illness or report of a missing person alert of a Senior Officer Potential threat of Pandemic virus/disease outbreak with Human to Human infection capabilities

Convening The IST IST Chair makes the call based on site needs Team members notified of first meeting time and location. Typically a bridge call. Multiple back-up locations have been identified and prepared for team requirements Meet and break process Alternates have been assigned for IST chair and all other team members to ensure 24 x 7 coverage
