2. Keep line material inside of truck bins to prevent falling off and striking a vehicle possibly following the truck

5. No label on herbicide spray bottle and unapproved gas cans. Also no cover on chainsaw bar.

6. Anytime chainsaw chaps have been cut they should be replaced as soon as possible.

8. No rate limit on lifting tongs. Recommend not using them for lifting poles.

9. No label on jug as to the contents of what's inside.

10. Hotline pruning stick test dates are out of date. They should be tagged “Out Of Service” until they are retested.

11. Flammables should be kept in flammable cabinet…too many items on shelf. Shelf has no weight rating listed on it.

12. No signage by drill press requiring PPE to be worn when operating. Also no PPE present near drill press.

13. Face shield and safety glasses must be worn when operating grinder. The condition of the face shield and safety glasses are very dirty. It is obvious they have not been worn for sometime.

14. Reminder to change out your AED pads.

15. Recommend replacement of lifting sling due to broken strands.

18. Damaged winch line. And safety latch missing from hook. Recommend replacing them both. Also winch line should have a braided eye and not a knot tied in it.

20. Oil leak and small spill recommend clean up ASAP.

21. Recommend taller pole stops for this pole bunker. Poles are taller than the stops.

22. Fuel spillage at fueling station at office.

24. Materials should not over hang shelving.

25. Recommend improving Universal Waste storage area. Provide containers, labels and appropriate PPE. Bulbs may get broken creating an environmental hazard.

26. Overhead hoist not inspected.

27. Ladder needs to be secured.

28. Seat belt is not out and being readily used.

29. These items should not be kept in the cab of this truck. Extremely dangerous to employee in the event of vehicle accident or roll over.

30. Unsecured materials in this truck.

31. Transformer not secured in back of truck.

32. Materials stored in PCB area other than oil. Suggest moving to other location.

33. No wheel chalks.

34. Transformer indicated PCBs should be kept in containment until tested. Indicate testing date and mange per EPA regulations.

35. No chalks on wire reels.

36. Recommend replacing rubber blanket storage tube cover, rubber blanket is dirty from road dust.

37. No weight rating on lifting sling. Recommend separating synthetic fiber equipment from chains and hoist.

38. Per OSHA copper hooks are not allowed on lineman belts. Replace with approved plastic break away hook.

39. Recommend reorganization of this truck bin.

40. No welding goggles or face mask with cutting torch.
