No label on herbicide spray bottle and unapproved gas cans. Also no cover on chainsaw bar.

Anytime chainsaw chaps have been cut they should be replaced as soon as possible.

No rate limit on lifting tongs. Recommend not using them for lifting poles.

No label on jug as to the contents of what's inside.

Hotline pruning stick test dates are out of date. They should be tagged “Out Of Service” until they are retested.

Flammables should be kept in flammable cabinet…too many items on shelf. Shelf has no weight rating listed on it.

No signage by drill press requiring PPE to be worn when operating. Also no PPE present near drill press.

Face shield and safety glasses must be worn when operating grinder. The condition of the face shield and safety glasses are very dirty. It is obvious they have not been worn for sometime.

Recommend replacement of lifting sling due to broken strands.

No weight rating on lifting sling. Recommend separating synthetic fiber equipment from chains and hoist.

Per OSHA copper hooks are not allowed on lineman belts. Replace with approved plastic break away hook.

Recommend reorganization of this truck bin.
