3 Community Impact is the process of identifying, addressing and solving issues that no single donor, charity or government agency can solve alone.

4  Is your agency listed below? (see Attachment A for more details) Big Brothers/Big Sisters, Boys and Girls Club of Manatee County, Education Consultants Consortium, Manatee Community Action Agency, Myakka Community Center, United Community Center, Early Learning Coalition  Are you willing to track specified outcomes using a standard measuring tool? If you answered Yes to BOTH questions, complete the Continuation of Funding Request Form – (see Attachment A)

5  What is Out-of School Time (OST)? (see Attachment A for more details) ◦ Extended time not in school ( Winter Break, Spring Break, Summer Break)  Do you have a program that addresses reading skills for preK-3 rd grade during OST?  Are you willing to track specified outcomes using a standard measuring tool? If you answered Yes to BOTH questions, complete application in ODM for funding consideration

6  Do you have a program that addresses INCOME? (see Attachment B for more details)  Job Training Initiatives  Financial Initiatives  Basic Needs Initiatives  Are you willing to track specified outcomes? If you answered Yes to BOTH questions, use the handout to determine the objective(s) and outcome(s) your program will address  Enter that objective (s) and outcome(s) in ODM – Program Description Summary - Questions 2 and 3 ‘The need statement or problem this program will address’ and ‘the program outcome or expected benefit or results for clients’

7  Do you have a program that addresses HEALTH? (see Attachment C for more details)  Safety  Access to dental care and/or education for children  Healthy meals for seniors and families  Are you willing to track specified outcomes? If you answered Yes to BOTH questions, use the handout to determine the objective(s) and outcome(s) your program will address  Enter that objective (s) and outcome(s) in ODM – Program Description Summary - Questions 2 and 3 ‘The need statement or problem this program will address’ and ‘the program outcome or expected benefit or results for clients’

8  Only submit applications for programs meeting the specified criteria  Grant amounts ◦ Education  Currently funded agencies – up to current funding  Out-of-School Time (OST) - up to $20,000 ◦ Income – up to $30,000 ◦ Health – up to $20,000  Dates to remember: ◦ February 16, 2016 – ODM Closes ◦ May 27, 2016 – UWMC Notification to agencies
