3 3 What is a Transfer Review A Transfer Review is a process of assessment that will consider if a child or young person needs an EHCP to meet their needs The assessment should be done over time in a person centered way The process must include a meeting to which parents/carers and the young person are invited Advice from Health, Social Care, Education and Psychology must be requested if needed (and must not be requested if not needed) It is an 20 week process which includes a 2 week notice period for parents and carers (18 weeks for the assessment)

8 8 Pre-statutory phase Meet with EP and create an action plan for each cyp with a Statement of SEN in your school Consider all the information you have for the child/young person collate the reports you have then decide if needs have changed Discuss with social care and health colleagues if involved and required Discuss with child or young person and parents/carers

More information can be found via the ELO www.essexlocaloffer.org.uk/content/one-plan-project Consider what information you have that will help complete the Needs Assessment report and If an Annual Review is required before the Transfer Review use it as an opportunity to do some of the work 9

10 Before the 18 weeks starts schools undertake some preparation with parents It is important that the most recent information is available and that this can be shared with the parent, young person and practitioners to support them in deciding if the information is sufficient and satisfactory for the transfer to an Education, Health and Care (EHC) plan. 10

11 In deciding whether existing advice is sufficient, it is likely that the following will be considered: how recently advice was provided; whether and how far the needs of the child or young person have changed since it was given; and whether it is sufficiently focused on the outcomes sought for the child or young person. 11

12 Thinking about requesting new advice It is important to note that additional advice must not be requested if such advice has previously been provided for any purpose and the person providing that advice, the local authority and the child’s parents or the young person are satisfied that it is sufficient for the purposes of an EHC needs assessment. 12

13 The Statutory Phase This is the statutory phase which should last no more than 18 weeks (20 weeks including 2 weeks notice period of the transfer review process) The EP will have discussed and agreed the start date with you by April 2016. The LA will then write to parents by the end of August 2016 informing them of the agreed start date Parents/carers and the young person must be invited to a meeting to contribute to the transfer review. This meeting will be approximately 8 weeks after the start date of the transfer review process. Parents/carers and young person should be given a minimum of 2 weeks notice of this meeting date.. 13r

14 The process Send a letter requesting new advice if agreed that it is needed and has not already been obtained. The new advice should be received 6 weeks after the request. All information to be used at the Transfer Review meeting should be circulated to parents/carer and the young person 2 weeks before the meeting Consider the information that is already available and how it can be used to populate the Needs Assessment Report. Ensure that the child or young person and their families have contributed and fully engaged with the process. 14

15 If new advice is being requested then follow these timescales At least 2 weeks before the process begins parents/carers and young person should be informed of the start date for the Transfer Review Week 1Send the letter to formally request advice for a Transfer Review, if this is required Week 6 Circulate the information in preparation for the meeting Week 8 Hold the Transfer Review Meeting and complete and agree the Needs Assessment Report Week 8 - 10 Return the paperwork to SAS Week 10 - 12 LA determines if an EHC plan is needed and creates a draft plan as required Week 12 – 18 LA consults with parents and finalises the plan 15

16 If no new advice is being requested then follow these timescales At least 2 weeks before the process begins parents/carers and young person should be informed of the start date for the Transfer Review Week 1Hold the meeting and complete and agree final version of the Needs Assessment Report with parents/carers and young person Week 2-4Return the paperwork to SAS Week 4-12 LA determines if an EHC plan is needed and creates a draft plan as require Week 12-18LA consults with parents/carers and young person and finalises the plan 16

17 17 How to do it You need to remember the following: Parents/carers and young person need at least 2 weeks notice of the start of the transfer review process Parents/carers and young person need at least 2 weeks notice of the date of the transfer review meeting 6 weeks is required if you are formally requesting advice; documents should be circulated 2 weeks before the transfer review meeting; all papers need to be returned to the Area Office within 10 working days of the transfer review Meeting.

18 The Transfer Review Meeting 1. Welcome and Introductions 2. Purpose of the meeting 3. Facilitated discussion focusing on: What people like and admire about the child or young person? What is important to the child or young person, from their perspective? What is important for the child or young person, from others’ perspective? What is working well? What is not working well? What progress has been made as evidenced in the educational setting’s written report? Any other issues 18

19 During the meeting Look at the Needs Assessment Report and agree what to keep and what to add? Look at the current Statement of SEN what is still relevant and what is not? 19

20 After the meeting Complete the Needs Assessment Report in WORD Send all documents to the relevant area office within 10 working days of the meeting. Add copies of all supporting documents ( eg.reports, One Page Profiles, annotated copy of current Statement of SEN) 20

21 21 Requesting Advice This is the letter to officially request statutory advice if it is needed. It should be sent in week 1 at the start of the transfer review process.Ensure reports are sent back to school in time for circulation before the Transfer Review Meeting giving professionals 6 weeks to return their advice. You do not need to send this if you already have: reports; and a clear picture of the child/young person’s needs; gathered or are gathering new reports before the statutory clock has begun. 22 22 Needs Assessment Report This is the document you will send to the Area Office after the Transfer Review Meeting. It should summarise all the information about the child and young person. Including all the evidence. If must include needs, outcomes and provision You should start to add information to the report as soon as you get it, do not wait until the meeting. At the meeting you should discuss the report and change it in-line with the advice, reports and parents/young persons views.

24 Some answers The LA write the EHCP they hold the legal responsibility They need to: decide if a plan is needed inform parents and consult on the plan if required Having a transfer review determines if an EHC plan is needed. If no EHC plan is needed the information is still valuable and will be very useful (Needs Report will be sent on) The Local Offer has information for parents and is being updated to support preparation for Transfer Reviews – iSEssex can help The transfer review can replace the annual review if timescales permit- when timescales do not permit use the annual review as a way of gathering the information you need for the transfer review Don’t call year 5 annual reviews early (only hold an AR when it is required) 24
