DEA 222 form no markouts or changes allowed--use new form pharmacy keeps blue form green copy to DEA brown copy to supplier electronic 222 form being used. supplier must ship all drugs within 60 days from written date if dr wants sch 2 drugs in his office…dr fills 222 form and pharmacy is supplier.

drugs received 3 files needed-----OTC and noncontrolled meds, sched 3-5 meds, and sch 2 meds. check box against the invoice invoice checked by pharmacist and signed sch 2 drugs--will come with supplier 222 form. get pharmacys 222 paper, initialed by pharmacist at drug received line. when all are counted, diagonal line across paper and RPh signs. store both papers in sch 2 file. $10,000 fine for no RPh initial on paper. if e-222 form print out hardcopy. let RPh handle sch 2 boxes.

Drug maintenance maker knows drug decomposition rate to give expiration date this date is good for recommended temp and conditions. degrades quicker if high heat, ect dehydrated meds----don’t last long after reconstituted. antibiotics liquids expire at 14 days except Augmentin and Biaxin expire at 10 days. most in refrig except clarithromycin and clindamycin best at room temp
