1 PCH Security Resident Badge Request

2 Residents Badge You must contact the security office through email with a list of on coming residents that need a badge. This can be done Monthly or Weekly. EMAIL: PCHbadgeaccess@imail.orgPCHbadgeaccess@imail.org Provide Full name & credential, School, Rotation Date (if applicable), and End of Residency with PCH date. The Residents can come and get a badge M-F 8am-4pm weekly excluding Intermountain Scheduled Holidays and unless otherwise posted. You can call us to make sure we will be open at 801-662-1020 The Resident no longer needs to bring the medical record form, but they need to bring the new PCH Resident Badge form.

3 PCH Resident Badge Form Form must be completed, signed, and brought to the security office when the Resident is getting a badge. Name on Form will be checked against the emailed list. Without both no badge will be issued. Make sure that form is completely filled out and signed by the Resident and the Department Coordinator. Only appropriate signatures will be accepted please do not have others sign in place of the Department Coordinator unless otherwise authorized by Security.
