4 2 Self-Determination Self Determination is a fundamental human right. It means that people, in partnership with family and friends, are empowered to plan and direct how they will live their lives. That people have the freedom to choose the services and supports they need, authority to control limited resources, and take responsibility for the decisions they make.

8 General Information 6 Who is eligible for Developmental Programs services? An individual is eligible to receive services if:  An individual has a diagnosis of intellectual disability before the age of 22 and  Has significant functional limitations in two or more skill areas and  Has scored 70 or below on a standardized intelligence test.

9 7 Centralized Intake Family/Individual contacts the County Intake Unit Gathers Information Confirms DP Eligibility Meets with Individual and Family Case transferred to BSU for Support Coordination

10 8 Developmental Programs Supports Coordination 090 County Supports Coordination Unit (Pennhurst Class Members Only) 092 Lenape Valley Foundation 093 Penndel Mental Health (DP Services only) 095 Penn Foundation
