3 Childhood Immunizations On-Site Clinics GOAL: 95% of all children age 0-4 will by fully immunized by 2010

4 Well Child Examinations GOAL: Increase number of children receiving well-child exams.

5 Oral Health Partnership with Dr. Frizzo – 47 surgeries completed in 2007 GOAL: 10% reduction in referrals.

6 Pre-School Preparation GOAL: Increase number of children age 0-5 who participate in quality, affordable pre-school education programs.

7 Teen Pregnancies Service Learning established as a Gateway Asset Health curriculum added for 7 th grade Teen Pregnancy Care Manager and other mentoring programs. Social Workers taking teen parents to health classes Asset surveys can now be tracked by student over time As of March, 2008 all pregnant or parenting teens were on track to graduate GOAL: 10% reduction by 2010.

8 Promoting Healthy Weight GOAL: Decrease 11-15 year olds with BMI exceeding normal. 4 th Grade Challenge Partnership between the St. Joseph School District, Dr. Jane Schwabe, Wellness Connections, and Heartland Points for participation Quarterly point redemption events Family activities (walking trail, cooking classes, etc.)

10 Youth Mental Health GOAL: Increase percent of youth with 21 or more developmental assets. Transition Plan Community based program for children age 14-18 years who require acute Inpatient Psychiatric Treatment To be implemented during 2008- 09 school year A re-entry coach within school setting to support these students and give guidance to transition back into the school setting
