Hyperacute stroke unit at Wycombe - offers intensive care, rehabilitation and therapy for three days following a stroke Information and Support -Prevention of social isolation, Links to local communities and local care Prevention Matters/Community Practice Workers – Integrated approach to catching vulnerable adults before personal health or social problems become critical Cohesive approach for medical care in hospital and when discharged MUDAS ( medical day unit), Adult Community Health Team in hospitals supporting discharge Dementia Care – greater information and innovative approaches Developing memory services – Dementia Support Workers for each Locality Social Services and GPs need to work togetherMulti Agency Group Meetings in Practices – (bringing together different agencies to discuss and help vulnerable patients) Use of “call for help” aids for elderly living at homeCall for help aids available for all through Bucks County Council.
