MISSION STATEMENT The purpose of this organization is to provide safe, quality and cost- effective care to diabetic patients, families and the community. We will strive to teach awareness, self-management of diabetes and prevention of this illness

VISION We will be the nation's leading diabetic care center by 2025.

PHILOSOPHY At Western Michigan Diabetic Center, we believe that each patient is unique, and we will strive to provide care to meet their physical, medical, and emotional needs. Our patients will be provided with information needed for independent management of their health to promote healing and prevent further injuries to their health.

CEO The CEO is responsible for overseeing the Diabetes Clinic Physicians The physicians are responsible for managing the patients’ health. They will use all areas of expertise to make a comprehensive health plan for all patients. WEST MICHIGAN DIABETIC CARE CENTER TEAM

Nurse Practioner The NP will treat and manage patients in coordination with the physicians in the clinic. The NP will assist with coordination of care as well as educate patients. WEST MICHIGAN DIABETIC CARE CENTER TEAM

Registered Nurse RN/Wound Nurse, preferably a certified diabetic educator: This RN will be responsible for community promotion of the clinic, advertising and fundraising/grant writing, and budget. The RN will be responsible for arranging a meeting place and obtaining the necessary supplies to run the clinic. The RN will oversee the scheduling of the patients and will arrange for referrals for patients needing them. The RN will assess and monitor patients and will take appropriate actions that will prevent further complications from the disease. The RN will educate and coordinate care of interdisciplinary services and help identify any other needs of the patient.

Medical Support Staff Responsible for overseeing the clinic in regards to answering phone calls, making appointments, ordering office supplies, and other duties as assigned. WEST MICHIGAN DIABETIC CARE CENTER TEAM

Counselors/Social Workers Counselors/Social Workers will identify needs of patients and help those in need get the appropriate supplies and education needed to prevent complications. They will work with insurance companies and help the patient navigate the healthcare system. Finance The finance director will set up yearly budgets and help maintain that budget. MARKETING STRATEGIES

The WMDCC will market our services to area hospitals and through community programs. We contact area primary care physicians making them aware of the services offered at the WMDCC. MARKETING

2014 ANNUAL BUDGET Expenditures for staffing, operating costs and education seminars are needed to provide safe quality diabetes care. Multiple funding will be acquired from other foundations to support the operation of the clinic and clinical research for evidence -based care. Maintaining the budget will aid in expansion over time.
