Aims To update all theatre personnel on the current WHO Checklist. To ensure consistency when performing WHO Checklist. Allow theatre personnel to raise concerns or suggestions.

Objectives To talk through each section of WHO Checklist. To ensure clarity of: Sign In Time Out Sign Out To address any concerns and document suggestions.

What are Never Events? Never Events are serious, largely preventable patient safety incidents that should not occur if the available preventative measures have been implemented. National Patient Safety Agency 2008

This section must be carried out in the Anaesthetic room, General Anaesthetic Anaesthetist and ODP The surgeon can choose to be present. Local/Regional Anaesthetic Surgeon and ODP Discretion can be used for questions five and six. All sections must be completed and signed by the ODP before Time out

All team members must be present, and silent cockpit observed. All team members must introduce themselves. Names and roles must be written on the whiteboard. This should be repeated if the team changes, and the whiteboard altered accordingly. All boxes must be ticked to indicate they have been read and asked out loud. Boxes 13,14, 15, 16, 17 need a tick to indicate the correct answer. Time out must be signed by a registered practitioner.

. Senior operating surgeon must sign Confirming that the Checklist has been entirely completed.
