2 Health Care Consortium Public Health Dept. led  Systems-focused information and discussion forum covering: 1.health care access, 2.health care utilization, 3.prevention efforts, 4.social determinants of health, 5.health equity, etc.  Meetings planned in 2015: May 7 th, June 25 th, July 23 rd, Sept. 24 th Nov. 19 th, and Jan 28, 2016 (Thursdays)  Joel Diringer facilitating/Public Health staff supporting  11:30 to 1:30 meeting time with lunch provided at the Public Health Dept. Outreach and Education Subcommittee Public Health and Community led  Community-based action-focused covering: 1)Trainings to enhance community actions to increase health care access; 2)Consumer messaging to increase health care access and improve health care utilization; 3)Information sharing and activity coordination to increase health care access (i.e., AB82/SB18, Covered CA, Health4All)  Crissy Gallardo and Katy Oestman facilitating/Katy Oestman supporting  Six or more meetings planning in 2015- 16 starting on May 22, 2015. Schedule TBD.  11:30 to 1:30 meeting time with lunch provided at the Public Health Dept. Prevention Action Team BHC led  Health4All Initiative focused to generate local support – at all levels – for health coverage for all Californians.  Crissy Gallardo and Isai Palma facilitating PAT and O & E connect with the Health Care Consortium through information updates, discussion and idea generation.
