Situation statement The health of the people of Bradford is generally worse than the England average Life expectancy is lower than the England average Life expectancy is 9.6 years lower for men and 8 years lower for women in the most deprived areas of Bradford than in the least deprived areas.

Priorities To increase healthy life expectancy of the people of Bradford Reduce differences in life expectancy and healthy life expectancy between communities across Bradford

Better Health, Better Lives Provides a framework to overview the New Deal Outcomes Total population, all age Much of the activity to support this outcome already in train

Work Programme Integration, Transformation and Sustainability New Models of Health and Social Care – Complex Care and Out of Hospital Care Development of Accountable Care Organisation to deliver improved health and social care. Introduction of Social Care Navigators to support people in making the best use of all resources available to them Enhanced Health Care in Care Homes – expand the use of telehealth/telemedicine Well Bradford – community based initiative to improve healthy life styles

Work Programme Adult Social Care Transformation Programme – developing a new social care offer to continue to provide good services and reduce the current cost of these by £23 million over the next two years. Self Care Programme – self care packs for dementia, heart failure, asthma and chronic obstructive pulmonary disease. Getting active in the community (volunteering) Keeping fit and active Watching out for people in your neighbourhood
