3 South Kent Coast CCG SKC CCG What is South Kent Coast CCG? - membership organisation of 31 GP practices in Shepway, Dover and Deal serving a population of 200,000. What are SKC CCG’s responsibilities? - buying (commissioning) the majority health services for the population. - monitoring the quality of the services delivered. Who are SKC CCG’s partners? - providers of hospital, community and mental health services. Local Authorities, Public Health, public and voluntary sector.

4 Where does SKC CCG spend the health budget?

5 The challenges Rapidly changing population (21% of population aged 65+ years – highest in Kent and Medway). Subsequent rises long term conditions ( e.g. coronary heart disease, diabetes). Health inequalities – significant difference in age expectancy across the CCG (13 years) Avoiding unnecessary hospital admissions. Providing care closer to home

9 PMCF - Background Almost a year ago GP’s in South Kent Coast joined Invicta Health They wanted to: Collaborate to improve services Attract new GP’s to work in the area Invicta Health is: A not-for-profit community interest company Started in 2008 Owned by GP practices in East Kent

10 PMCF – The Bid £50 million was made available by the Prime Minister to: Improve access to general practice (7 day working) Develop new ways of providing GP services We decided to bid to see if we could fund major changes in SKC. There were almost 250 bids of which 20 were selected. The aim is for rapid changes that will have an impact in the next 12 months.

11 PMCF – Plans: To create a primary care hub in Folkestone at the Royal Victoria Hospital To be open 8am to 8pm seven days a week Patients clinical records will be accessible via a linked computer system Routine and urgent care – both booked and walk in Patients can use the hub like a branch surgery of any of the local practices

14 PMCF – Plans continued: We plan to include other services: Primary care mental health assessments Paramedic urgent visiting Access to temporary care home beds Improved links to community and social care

15 PMCF – Working with: GP practices CCG East Kent Hospitals Ambulance Trust Community Trust Social Services Mental Health Trust NHS England The Prime Minister!

Current October 2014 Open 7 days a week 8am – 8pm No appointment necessary Treats minor injuries (minor illnesses will be redirected on site to PMCF GP) Nurse Practitioners X-ray *Mon - Fri 8am – 8pm (*working with EKHUFT to extend) Open 7 days a week Mon – Fri 7am - 10pm W/E’s & BH 9am – 10pm No appointment necessary Treats minor injuries and minor illnesses Nurse Practitioners X- ray Mon - Fri 9am – 5pm Creates equal access to Minor Injury services across SKC CCG

19 The public’s role Help design: a high quality, affordable vision for joined up care services in Folkestone for the future. a model for out of hospital services centred around a community “hub”, making best use of the Walk in Centre and our local primary care services “what services will I need in the future and how will I access them”?
