4 © Finnish Institute of Occupational Health – www.ttl.fi 6.6.20164 The OH Services Act defines competence requirements The Ministry of Social Affairs and Health has determined competence requirements for occupational health services professionals on the basis of Section 1383/2001 of the Occupational Health Services Act (Handbooks of the Ministry of Social Affairs and Health, ISSN 1236-116X; 2004:11; ISBN 952-00-1545-0). The activities of the occupational health care professionals and experts must comply with the provisions on the content and tasks of occupational health care laid down in the relevant legislation (1383/2001; section 12). Good occupational health care practice presupposes multidisciplinary co-operation by experts and multiprofessional knowledge of the relationship between work and health and its management. The training given is based on FIOH’s research and development activities. The training activities are guided by expertise in the spheres of health, the work environment, and work communities and familiarity with the practical operational models combining them Owe Österbacka

They: can assess their clients' physical and mental well-being status know the necessary health education and communication methods are able to support and enhance their customers' work ability can give advice on the rehabilitation service options Work-related illnesses and occupational diseases Participants: know the Finnish legislation concerning occupational diseases know the basics of epidemiology can recognize the symptoms and pathology of occupational diseases Owe Österbacka

The courses are meant for professionals and experts in occupational health and safety, for labour protection personnel, and for people working in personnel administration and production. 16 © Finnish Institute of Occupational Health – www.ttl.fi Maintaining professional skills The employer of an occupational health care professional or expert has a duty to ensure that this person attends continuing education to maintain his professional skills sufficiently often and no less than once every three years. The continuing education duty also concerns a health care professional engaged in occupational health care as an independent professional. The Ministry of Social Affairs and Health may issue instructions on the content, quality, amount and organization of continuing education. 17 © Finnish Institute of Occupational Health – www.ttl.fi General information on Specialist physician’s training in occupational health care – 6 years The Specialist Physician’s Degree Programme in Occupational Health Care consists of a basic and specialised qualification. The degree programme is six years in length. The basic qualification requires two years of training (for more details, see Additional Training in Primary Health Care). Service during the four-year specialised qualification includes sections on occupational health care, assessment of work ability and rehabilitation, clinical rotations in other fields of medicine, and a rotation at the Finnish Institute of Occupational Health. In accordance with university regulations, the degree programme also includes multiprofessional management training. An agreement between the training site and the university must be in place prior to the start of training. The training site will appoint a personal instructor, who is a trainer approved by the university. This trainer is responsible for the implementation of the study programme. He or she guides the professional growth of the specialising trainee, monitors the implementation of the trainee’s personal specialisation plan, and assesses the training together with the specialising trainee.

18 © Finnish Institute of Occupational Health – www.ttl.fi Specialist physician’s training in occupational health care Basic qualifications training, 2 years The overall objective of this training is to prepare the individual for work as a primary health care physician/a licensed physician with the rights of a general practitioner. Goals of the basic qualifications training The basic qualification training must include at least nine months of service in a municipal health centre and at least six months of hospital service (for more precise information, see the heading Additional Training in Primary Health Care). At most six months may be served at some other university- approved public or private health care institution or unit, performing general practitioner-oriented duties.

19 © Finnish Institute of Occupational Health – www.ttl.fi Specialist physician’s training in occupational health care Specialised qualification training, 4 years Specialised qualification training comprises rotations in occupational health care, in work ability and rehabilitation, in relevant clinical disciplines, and at the Finnish Institute of Occupational Health. The specialised qualification training includes both in-service and theoretical training.

20 © Finnish Institute of Occupational Health – www.ttl.fi Specialist physician’s training in occupational health care Specialised qualification training, 4 years Occupational health care unit (24 months) The goal of this unit is familiarisation with the occupational health physician’s profession. The specialising trainee will be able to apply the theoretical base of knowledge in occupational health care and will have a thorough understanding of occupational health care service processes and multiprofessional activities. Following completion of this unit, the specialising trainee will be able to act as an expert with the workplace and other relevant entities, in accordance with good occupational health care practice.

21 © Finnish Institute of Occupational Health – www.ttl.fi Specialist physician’s training in occupational health care Specialised qualification training, 4 years Assessment of work ability and rehabilitation unit (6 months) The goal of this unit is to deepen skills in assessing work ability,functional ability, and the rehabilitation of working-aged people. The unit is completed at a university-approved training site where work ability and working-aged people’s rehabilitation needs are assessed. For the specialising trainee, the training site will appoint a personal instructor approved by the university as a trainer.

22 © Finnish Institute of Occupational Health – www.ttl.fi Specialist physician’s training in occupational health care Specialised qualification training, 4 years Clinical service in other fields of medicine (12 months) Clinical service in other fields of medicine must support the goals of training and the implementation of the specialising trainee’s personal specialisation plan.During this unit, knowledge of the most important diseases among working-aged people in primary health care is deepened and, for the purposes differential diagnosis,, knowledge of diseases which may be work-related is enhanced. Clinical service is completed in at least three applicable fields of specialisation (at least two fields of specialisation for those granted study rights before 1 August 2003). At least three months of service is required in each field of specialisation. Suitable fields of specialisation are psychiatry, physiatrics, dermatology, pulmonary diseases, oto-rhino-laryngology, internal medicine, surgery and neurology. It is recommended that the main emphasis be on outpatient work. In exceptional circumstances, the person responsible may approve clinical service in another field of specialisation.

23 © Finnish Institute of Occupational Health – www.ttl.fi Specialist physician’s training in occupational health care Specialised qualification training, 4 years Finnish Institute of Occupational Health Unit (6 months) This segment concentrates on occupational medicine,applied research in occupational health care, and introduces the principles of scientific research. It is recommended that this unit be completed at the end of specialisation. The unit at the Finnish Institute of Occupational Health can be completed within the Institute’s branches throughout the country. At some sites, it can also be completed at university hospital or central hospital settings, on the condition that the university has approved this service as a replacement for the Finnish Institute of Occupational Health.
