1 Questionnaire on health services that look after you when you are pregnant and after your baby is born. We want to know what you think about the health services that look after you when you are pregnant and after your baby is born. This questionnaire has 3 parts. Part 1 is questions about health services that look after you when you are pregnant and after your baby is born. We want to know what you think. Part 2 is for any women to answer that can have a child. Part 3 is a few questions about you. Some people will need someone to support them to go through this information so that they understand it.

2 Part 1 is questions about health services that look after you when you are pregnant and after your baby is born. We want to know what you think. These questions can be answered by everybody. How important do you think it is to have midwife clinics close to your home? A midwife clinic is a place where you can see nurses and doctors who know all about pregnancy, babies and giving birth. 3 3. How important do you think it is to have scans before you give birth close to your home? Scans are tests nurses do to see if your baby is healthy. 4. Sometimes you may need to see a midwife to check the health of you and your baby before your baby is born. A midwife is a nurse who knows all about caring for you and your baby through pregnancy and afterwards. How important do you think it is to be able to see them at opening hours different to 9:00am to 5:00pm, Monday to Friday? 5 7. Sometimes you may need to see a midwife to check your health and the health of your baby after it is born. A midwife is a nurse who knows all about caring for you and your baby though pregnancy and afterwards. How important do you think it is to have doctors, midwives and medical equipment close by when you have your baby? Medical equipment can be things like: A monitor to hear your babies heart beat. 6 9. How important do you think it is that you are given information about things that can happen to you or your baby ? This can be information like: What will happen if you baby is early 10. 8 Please tick a box for each of these questions. This part of the questionnaire is about getting to a clinic to see a midwife. A midwife is a nurse who knows all about caring for you and your baby through pregnancy and afterwards. We want to know how far you would be willing to travel to see a midwife. 13. To go to a midwife clinic I will be happy to travel as far as 5 miles5 to 10 miles 11 to 25 miles 50 miles and more 25 to 50 miles Part 2 This part of the questionnaire is for women who are at an age where they can have babies. You do not have to fill them in if you are not at an age where you can have children.

10 16. If your doctor says it is safe, how important is it to you to give birth at home? 17. Sometimes if there are problems you or your baby will need extra help. You may need help: from baby doctors, with controlling the pain or with having your baby. How important is it to you that you are able to have these close by. 11 18. You have the choice to have your baby where you want to. In some places baby doctors and extra support might not be available straight away.
