1 Face to Face (F2F) Documentation Changes for Physicians

2 Situation CMS revised the home health face-to-face (F2F) requirements for certifying patients for home health services. Goal is to promote care coordination and patient safety Current F2F documentation (paper and PowerNote are not sufficient to meet new CMS requirements) Probe/educate audits begin 10/1/15 (retro to 8/1/15)

3 F2F Documentation Options There are 3 options for completing F2F documentation: –Paper –Workable PDF’s –CareConnection (Advocate hospitals only) We will not be accepting the old paper form after November 30 th, 2015. Face to Face information, copies of paper tools and workable PDF’s can be downloaded at: www.advocatehealth.com/physiciandocumentation www.advocatehealth.com/physiciandocumentation

4 F2F Documentation Changes Advocate Medical Staff: F2F Certification Statement and Progress Note (supporting documentation) must be obtained within the first 7 days of referral (preferably at time of referral). Non-Advocate physicians: F2F Certification Statement and Progress Note (supporting documentation) must be obtained at time of referral completion.

5 F2F Encounter Timing Progress Note (supporting documentation): can be completed by either a physician (MD, DO, DPM) or APN or PA to support the need for HH; if completed by an APN or PA, no physician co- signature is needed. Certification Statement: must be completed by a physician (MD, DO, DPM). The Certification Statement and Progress note must be completed with a couple days of each other.

7 Step 1 – It starts with the order F2F documentation is triggered from the Discharge Planning Eval Adult order when Home Care Evaluation is selected as the reason for Discharge Eval:

8 Step 2 – F2F Documentation Alert An alert will appear indicating patient has an active order for HH and F2F documentation is needed; click on links to go to documentation

9 Documenting Using PowerNote

10 Step 3 - Accessing PowerNotes 3. If ordering both Home Health and Home Oxygen, you must complete a PowerNote for both. 3. 11 Step 4 - Placing Note Correct Folder Under Type, Right Click to have the pop up box appear. Check the All Note Type list to be able to scroll and find the Home Health Certification Folder. Save the powernote as a precompleted favorite – the folder type will stick for the note if this is done. Under Type, Right Click to have the pop up box appear. 12 1. Choose Date and Time from Calendar 2. Complete Clinical Reason/Diagnosis for Home Health i.e. Diabetes, Wound Care 5. Identify outpatient physician managing patient 4. Choose One or More of the Home Care Interventions 3. Select One or More of the Homebound Reasons Step 5 – F2F PowerNote

13 F2F Documentation Location Completed F2F form will appear in the Physician Documentation folder > Home Health Certification

14 When Certification field is clicked, documentation is added to Physician’s Progress Note in Patient’s Care Connection Record PowerNote is Complete!

17 1. Enter the date the F2F Assessment was completed 2. Click “Yes” if you are physician following patient post-discharge or “No” if you are not 3. If you are not the physician following the patient post- discharge please enter in the physician who will follow the patient in the community Step 2 - F2F Certification Statement

19 Reference text is a resource guide of how to complete the F2F documentation Provides examples of eligibility criteria Summarizes home bound criteria examples (2 must be met) Step 4 – Resource Guide
