Known to be or reasonable suspected to be a terrorist 2. Known to or reasonable suspected to belong/support terrorist organization 3. Unable to verify individuals claimed identity 4. Individual has submitted fraudulent information concerning identity 4

Reasonable basis to believe individual will attempt to gain unauthorized access to classified, Privacy Act, proprietary, or other sensitive or protected information 6. Reasonable basis to believe individual will unlawfully or inappropriately use an access credential outside the workplace 7. Reasonable basis to believe that based on individual’s criminal or dishonest history that issuance of access credential poses an unacceptable risk to the installation/mission 8. Individual has/had been barred from entry/access to a Federal installation or facility 9. Individual is wanted by Federal or civil law enforcement, regardless of offense or violation 5

Individual has been incarcerated in prison within the last ten years, regardless of offense/violation, unless released on proof of innocence. NOTE: Suspend sentences may be allowed based on review by 90 SFS/CC. 11. Individual has any conviction for espionage, sabotage, treason, terrorism or murder 12. Individual’s name appears on any Federal or State agency’s “watch list” or “hit list” for criminal behavior or terrorist activity 13. Individual has been convicted of a firearms or explosive violation within the past ten years. 6

Individual has been convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell, or drug distribution. NOTE: Conviction of drug possessions with the intent to sell, drug distribution or drug manufacturing over 10 years ago may be allowed access. Access will be dependent on no other drug offenses or serious felony convictions have occurred since original conviction based on review by the 90 SFS/CC. 15. Reasonable basis to believe, based on an individual material or intentional false statement, deception, or fraud in connection with Federal or contract employment, that there is an unacceptable risk to the installation/mission. 7

Reasonable basis to believe, based on nature or duration of alcohol abuse, without evidence of substantial rehabilitation, that issuance of access credential poses an unacceptable risk to the installation/mission 17. Reasonable basis to believe, based on nature or duration of the illegal use of narcotics, drugs, or other controlled substances without evidence of substantial rehabilitation, that issuance of access credential poses an unacceptable risk to the installation/mission NOTE: Individuals with more than 5 drug convictions within the last 5 years will be denied access. 8

10 Ready to fight…Anytime, Anywhere UNCLASSIFIED Contractor Pass Request Procedures 5352.242-9000 Contractor Access to Air Force Installations CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (NOV 2012) (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, valid vehicle insurance certificate, and [insert any additional requirements to comply with local security procedures] to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with [insert any additional requirements to comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management] citing the appropriate paragraphs as applicable.AFI 31-101AFI 31-501 (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. 10

11 Ready to fight…Anytime, Anywhere UNCLASSIFIED Contractor Pass Request Procedures (Cont) Prime contractors must submit a separate letter on company letterhead stationary for each individual needing a Contractor ID Badge. A mandatory format is attached. It is preferred that the letter be either typed or in electronic format. When a Contractor Pass Request is filled out with personal identification information, per Air Force Instruction 33-332, the form in now considered to be “For Official Use Only” and steps need to be taken to ensure the security and confidentiality of the information. This information will be reviewed by the Contracting Officer and forwarded to Pass and Registration. Processing time for base ID badges is 2 to 3 days. Letters should be submitted several days in advance of when the employee is required on the job site. If the individual does not pickup pass within 30 days a new request is required to be submitted. 11

12 Ready to fight…Anytime, Anywhere UNCLASSIFIED Contractor Pass Request Procedures (Cont) When the ID card is ready, individuals must go to the visitor control center located outside of Gate 1 to be signed in and issued a temporary pass to gain access to the base. After the temporary pass is issued the employee must proceed directly to Pass and Registration to be issued their Contractor ID Badge. Employee must bring a State or Federal picture ID card as well as proof of their social security number in order to receive a Contractor ID Badge. If applicable, a Resident Alien card must also be presented. Contractor ID Badges must be carried by individual at all times and must be presented upon request of government inspectors or security forces personnel. Contractors found on base without a valid pass will be escorted off base and may be subject to administrative or criminal penalties. 12

13 Ready to fight…Anytime, Anywhere UNCLASSIFIED VEHICLE ACCESS PROCEDURES Contractors that will be driving a vehicle on base must provide registration and proof of insurance to register the vehicle when they are issued their contractor’s badge. The vehicle information will be entered into the computer system. Contractors may register a maximum of three vehicles into the system. If a vehicle used to gain access to the installation is emblazoned with the company name on the side of the vehicle, you will be required to enter through the vehicle search area at Gate 5. Insurance and registration must be in the vehicle at all times. 13

14 Ready to fight…Anytime, Anywhere UNCLASSIFIED RULES TO FOLLOW You are accessing a Federal Installation. Federal Laws and Regulations apply to any individual either trying access or on a Federal Installation. Marijuana is a Federally Controlled Substance! Seat Belt use in Mandatory! Wearing of any portable headphones, earphones, or other listening devices (except for hands-free phones) while operating a motor vehicle is prohibited! Antelope have free run of the base. They will walk into traffic! Speed limit is 30 MPH unless posted otherwise. 14
