No more than 5 years old- School code is 5816 GPA (.3.0) Personal Statement (No longer than 2 pages, double spaced - name on both sheets) For applicants from countries where English is not the first language TOEFL Scores when applicable (No more than 2 years old) and the TSE (Test of Spoken English) 6/7/20166

8 Standard III-A The applicant must have transcript credit (2 S.H. of course work, advanced placement, CLEP, or examination of equivalency) for each of the following areas: biological sciences human anatomy and physiology, neuroanatomy /neurophysiology, genetics physical sciences physics, inorganic or organic chemistry social/behavioral sciences psychology, sociology, anthropology mathematics non-remedial mathematic Courses in biological and physical sciences specifically related to communication sciences and disorders (CSD) may not be applied for certification purposes in this category 6/7/20168

21 Clinical Education ASHA certification requirements: »400 clock hours of supervised clinical experience 25 hours clinical observation 375 hours direct client/patient contact »Knowledge and Skills Acquisition (KASA): specified skill-based competencies in both evaluation and intervention with a variety of client age groups across the lifespan in the “big nine” areas of clinical practice (see next slide) »Demonstration of knowledge and skills in ethics and contemporary professional issues 6/7/201621
