3 Reasoning for an Annual Health Check -replacing the 3 year renewal process Supporting clubs to retain the high standards they achieved when applying for FA Charter Standard status Reduce the number of clubs losing their accreditation and re-writing a new application form (or not) Support clubs in tracking the validity of their volunteer’s qualifications Allow clubs to identify their annual coach education requirements Aligning coaches / managers to teams showing active coaches Providing clubs with an opportunity to identify the support they require from the County FA Introduce clubs to FA Member Services Provide consistent communication with the County FA Reward clubs each year with Umbro vouchers

4 What is the annual Health Check? A presentation of data held by the County FA on the club and the volunteers within the club showing for example: How many teams the club has Who the key personnel are within the club Who the managers and coaches are with the teams What qualifications the coaches have A number of questions asking how the club adheres to the FA charter Standard criteria for example: Is the club delivering the Respect Programme? Is a first aid kit present at training & fixtures? All FA Charter Standard clubs need to complete the Health Check unless they received their accreditation that season (from 1 st July)

5 We know there may be: Areas where clubs may need support to fulfil all the criteria Corrections required to the information presented Volunteer positions vacant New coaches without qualifications Coaches with qualifications that are not identified on the form or have their qualifications expired Coaches not aligned to teams What we ask of clubs is: To complete and return your health check form to your County FA to the best of your knowledge before the 31 st March 2011 To work with the County FA to address any gaps in the criteria This is the start of the process and all information will be retained for future years building you a comprehensive club profile.

6 Seasonal Calendar Months September to March County FAs to communicate the process to their FA Charter Standard clubs Clubs to receive or generate their Health Check form Clubs to complete their Health Check form and return to their County FA Clubs to plan to address any gaps in criteria & communicate these to the County FA March 31 st If the club has not contacted County FA their status will be suspended June 30thIf the club has not contacted County FA their status will expire

8 What do the Health Check forms look like? Different form dependant on the level of FA Charter Standard accreditation Contains unique information about the club Contains questions relating to the criteria specific for that level of accreditation Generated in a pdf format so will have to be completed in hard copy and returned to your County FA This is due to the complex nature of the report generation process and as a result not being able to create editable pdf’s

9 How to complete the Health Check form The following slides explain how clubs should complete the Health Check form once generated. The example used is for a FA Charter Standard Community Club so will be the most complex form you would see. Forms for other levels of accreditation would contain less detail as the criteria are less. If you are completing the process through FA Member Services this will allow clubs to alter and add detail to their Health Check form online prior to generating it - please refer to the guidance booklet how to do this.

11 Page 1 in more detail: Check the form is for the correct level of accreditation This could be Community, Development, Youth or Adult If this is NOT correct contact your County FA and they will get this changed and generate another form Check the Secretary and the CS Co-ordinator details correct If not please fill in the correct details in the box provided If there is no FA CS Co-ordinator listed it will say so

12 Page 1 Club Details The form will highlight the affiliation details for your club It is essential that you check the number of teams you have and that all the relevant affiliated sections of your club are included If any of these are incorrect write in the correct details and contact your County FA. It is essential the form represents the whole club If your club constitution or policies have changed please indicate so on the form and provide an updated copy to your County FA

13 Page 1 Club Welfare Officer (CWO) details All clubs with a youth team (Under 18) must have a CWO Check that the details of the CWO are correct and that they have an Accepted CRB check and have attended both the FA Safeguarding and Welfare Officer Workshop training. If the CWO details are incorrect please complete the correct details in the box below and contact your County FA Welfare Officer If any of the CRB or Workshop details are missing please contact your County FA Welfare Officer Don’t worry if the Safeguarding Workshop date was more than 3 years ago as the Welfare Officer Workshop supersedes this.

14 Page 2 in more detail: Review the voluntary roles within the club If the details are incorrect please write in the correct details If there are any key roles missing write these in the blank spaces The form will detail what positions are mandatory for FA Charter Standard Not all volunteers need to have an Accepted CRB check but it is good practice for all key positions

15 Page 2 Discipline The discipline record for the previous season and the current season will be displayed for the club to review their discipline record If you are a Community Club this page will also highlight the coach who has attended the FA Coach Mentoring course so they can support other coaches in the club If this is blank please complete with the coach’s name, date of birth and details of the course they have attended or plan to attend

16 Page 3 FA Charter Standard Criteria Check: Page 3 contains questions that identify how your club is adhering to the criteria. Please complete the form circling YES or NO where appropriate and providing information where requested. 17 Page 3 County FA Support To support your club in adhering to the FA Charter Standard criteria it is recognised that you may require assistance from the County FA. Please prioritise the key areas of support required The bottom of the form displays the postal address of your County FA where you are to send your completed form 1 2 3

19 Page 4(1) The qualifications Record This page is the most complex page as it will display information held on the each team and their coach / manager(s) We know there may be gaps in the information for example where : The team managers and coaches aren’t aligned to teams The information on coaching qualifications haven’t been aligned to that coach’s records The coach may have duplicate records Please try and complete this page as much as possible as this will update the information held on your club for future Health Checks The County FA will be able to help you on many areas of this so please do ask for their assistance. They may have copies of your original FA Charter Standard application with copies of coach’s certificates We recognise that to complete the page fully may take more than one season so try and make as best start as possible

20 Page 4(2) details: Step 1Check that all the teams for your club are listed If not please correct the form by either deleting or adding teams Step 2Check that the coaches/managers aligned to the correct team and their date of birth is correct If incorrect or a team has no coach/manager displayed please write in their name and date of birth. If you know their FAN number please include it here. If a team has more than one coach/manager they will all be displayed If a coach is aligned to the club but not a team their details will show with no team

21 Page 4(3) Coaches Qualifications The FA have only recently implemented an automated coaching qualification database. For this reason there may be qualified coaches whose records don’t show they have a qualification. Step 3If there are coaches/managers who’s qualifications are not showing please write in the following details: - Qualification Level e.g. L1 - Certificate number - Certificate date - Coach’s postcode With the above information the coach’s records will be updated Your County FA may be able to support you with these details

22 Page 4(4) FA Emergency Aid, Safeguarding As with the coaching qualifications there will be coaches who haven’t got their FA Emergency Aid qualifications displayed. Step 4Please write in the following information so the coach’s records can be updated: The Qualification e.g. FA emergency First Aid Certificate date The FA do hold comprehensive records of coaches who have attended the Safeguarding Children workshop Step 5 If the correct details are not displayed please write in the following: The Qualification e.g. Safeguarding Children Certificate date Certificate number Please check the expiry dates of the above courses as coaches may need to renew their awards

23 Page 4(5) Enhanced CRB checks Any coach working with a youth team (U18) must have an Accepted Enhanced CRB Check Step 6If this column is blank the coach needs to see the Club Welfare Officer (CWO) and complete a CRB application form If a coach has an Accepted CRB form and this is not displayed they need to contact their CWO with the following: Name & Date of Birth Address FAN number (if known) The Club Welfare Officer can then update the coach’s and club Record - The County Welfare Officer may be able to offer support

24 Page 4(final)Age Appropriate Coaching If a coach has attended an Age Appropriate Introductory course or the FA Youth Award Module 1 or 2 the date of the course should appear in the qualifications record Step 7If the coach has attended a course But the date is not displayed please write in the following details: The Qualification e.g. Youth Award Module 1 Certificate date FAN number (on certificate) With the above information the coach’s records will be updated– your County FA may be able to support you with these details
