Faulty gas fire—ignited by matches. Very dated and dilapidated electrical installation. Visited 4 times per day by carers. Numerous disabilities inc. dementia. Lived alone. Smoke alarms fitted by an “agency” but on top of the polystyrene tiles on every ceiling.
