1 Goodhue County HHS 2015 School-located Flu Vaccination Clinics Exercise Objectives: 1.Exercise medical countermeasure dispensing plan by providing flu vaccinations to children in schools. 2.Coordinate emergency response with key community partners: schools, HHS staff, MN Responds (MRC) and student volunteers. 3.Align the Incident Command System with our agency emergency response plans. 4.Determine effective methods to get information to parents. 5.Implement direct on-line registration. 6.Demonstrate management of vaccine inventory.

3 FREE FLU VACCINATIONS FOR ELEMENTARY STUDENTS AT SUNNYSIDE AND BURNSIDE SCHOOLS Goodhue County Health & Human Services will be offering free flu vaccinations to students in grades K-4 as part of an emergency preparedness grant. If you would like your child to receive a flu shot or flu nasal spray at school, please go to Red Wing School’s website for more information about how to register. Post your website here. Permission forms can be completed online or paper. Permission forms must be submitted by the dates listed. No exceptions. Sunnyside Clinic – Thursday, November 12, 1p.m. Submit form by Monday, November 9, 3:00P Burnside Clinic – Thursday, November 19, 1p.m. Submit form by Monday, November 16, 3:00PM For questions or assistance call 651-385-6100.

4 FREE FLU VACCINATIONS FOR STUDENTS AT PINE ISLAND SCHOOLS As a part of an emergency preparedness exercise Goodhue County Health & Human Services will be offering free flu vaccinations to all students age 18 and under at Pine Island schools on November 5 th. 5 FREE FLU VACCINATIONS AT SCHOOL November 5 th District Wide As part of an emergency preparedness exercise Goodhue County Health & Human Services will be offering free flu vaccinations to all students in Kindergarten – 12 th grade at Pine Island Public Schools on November 5 th. If you would like your child to receive a flu shot (IIV) or flu nasal spray (FluMist-LAIV) at school, please read these instructions carefully and review the information about flu vaccine at www.immunize.org/vis. Then complete the online form at the link listed below. You will have the option to receive a confirmation e-mail message.www.immunize.org/vis The online form is submitted via a secure site and only Public Health staff will have access to it. The online form can be completed in the following browsers:  Internet Explorer versions 8 through 11  Chrome  Firefox  Safari You may need to turn off Compatibility Mode if the form does not display. Click to complete the online Flu Vaccination FormFlu Vaccination Form Please complete one registration form (either online or paper) for each child you are requesting to vaccinate. Registration online is preferred. A paper form is available by request in the health offices. This should be completed and turned back into the health office by the deadline. NO Exceptions will be given. Paper Flu Clinic Form- Print and Return to the Health Office Paper Flu Clinic Form Submit the form at least 3 days before the clinic. Forms will not be accepted after Monday, November 1st at 3:00 PM. This is to assure that we have enough vaccinations for children. For questions or assistance call 651-385-6100. PLEASE NOTE: Because of delays in shipping, FluMist nasal vaccine may not be available. If your child qualified to get the mist and it is not available, please check the box on the form if you would like your child to get the injection instead. If mist is not available and this box is not checked, your child will not receive the flu immunization.

34 WHAT NOW? Keep the momentum going! Offer annual clinics to schools. Partner with a vaccinating agency with billing capacity. Collect insurance information on permission form and offer VFC vaccine to uninsured. Improve flu vaccination rates
