3. Client Satisfaction Survey An email will be triggered from MPPM when the SIP is not defined within specified timelines. Quality Maturity Index An email will be triggered from MPPM if QMI is not improved and not Green APPLICATIONS01 March 2013| 4 EWR Escalation Matrix If QMI is not improved by 5% AND not in Green for 2 monthsEscalation Email To : LT3 If QMI is not improved by 5% AND not in Green for 3 monthsEscalation Email To : LT2 If QMI is not improved by 5% AND not in Green for 4 monthsEscalation Email To : LT1

Metrics (Production Support) AOS Utilization & Productivity (Ticket / FTE) are displayed in "Colors for org metrics" Table Productivity FTE value will be calculated using the below formula and calculated FTE will be used in Productivity calculation – Productivity FTE Value = (Actual FTE - (Utilized hours for application change request /155) - (utilized hours for application monitoring /155)) – Target for productivity - If Productivity value <= 8 then ‘Yellow’ else Productivity color will be ‘Green’ Utilization value and color will be calculated using the below formula. – Utilization = ((Total number of incident fixed * 5.5/155+ Utilized hour application monitoring /155 + Utilized hours for application change request/155) /Actual FTE)* 100; – Target for Utilization - If Utilization value <= 60 then ‘Yellow’ else Utilization color will be ‘Green’ APPLICATIONS01 March 2013| 5

6. Audit Schedule Renamed the field - "Audit State Date" to "Audit Scheduled Date" & "Audit End Date" to "Audit Conducted Date" APPLICATIONS01 March 2013| 6

Project Management Requests and Workflow Deliverables/Milestones(Add all deliverables given in SOW) New fields to track customer approval for Completed Deliverables is added – Acceptance Criteria (Text Area) – Accepted/Approved by Client (Drop Down (Yes/No)). – Accepted/Approved Date (Date approved by Client) – Approval mail attached to Request? (Drop Down (Yes/No)). – Evidence to be attached to the request in reference section Mail notification triggered on Project Kick off action is changed to include apps.resue@mphasis.com in CC APPLICATIONS01 March 2013| 7
