II.Process Overview Process Overview (melting process ) Melting process –Materials used for melting processes are pig iron, scrap iron, and return cast iron. Once melting was started after starting of electric furnace, they did charging and inputting of recarburizer manually. 5 ~ 7 times of discharge was conducted a day. - Pig iron: pig iron is steelmaking and casting material. They produced by restoring iron component from iron ore in a shaft furnace (melting furnace). the content of carbon material is iron of 1.7 ~ 9.7%. - Return cast iron: reused iron from parts used for inlet hole for molten metal during casting. –Return cast iron processing was done at factory entrance. Return cast was temporarily stored at factory gateway and moved to near the melting furnace. Therefore, there was big possibility of influx of moisture such as rainwater. IV. Fatality Process At about 07:00, April O, OOOO the victim went to the office and guided to a co-worker who was doing melting work by factory superintendent at about 08:00. he was ordered to support the melting work. At about 08:40, the melting was completed for the first discharge and started the second charging. At about 10:20, the co-worker who was additionally charging pig iron into melting furnace moved to the storage place to transport pig iron and scrap iron. Fatality Process The victim ran down to forming workplace and fell down on the ground, Co-workers called 119 to take him to the hospital, The victim was suffering from 3-degree burn (80%) on the thigh, abdomen, and arm and died from burn complication. Fatality Cause Improper storage of raw materials –Since the raw materials such as scrap iron and return cast iron were stored outside of the workplace, there was vapor explosion hazard during melting owing to influx of moisture such as rainwater. Wrong inputting method of raw material –There was vapor explosion hazard during charging because he directly charge raw material with remaining moisture such as rainwater into melting furnace without drying. V. Fatality Cause No personal safety equipment such as heatproof clothes - In a place in where high heat objects are handled, you must wear heatproof clothes to prevent burn or other hazard. Improving storage method of raw materials –To prevent processing water or rainwater from leaking into the site, you must install partition walls or move the storage place for raw material (scrap iron, etc) to dry place. Improving inputting method of raw materials - Improving inputting method of raw material (refer to preheating system.). Before inputting raw material (scrap iron, etc) into a furnace, you must check for foreign materials such as moisture and charge raw materials after drying using preheating furnace before charging it into melting furnace. Precautions Personal safety equipment such as heatproof clothes - To handle high heat objects, you must ensure that you wear personal safety equipment such as heatproof clothes and heatproof gloves.
