8 Holistic Integrated Services Behavioral Health Home Information Sheet In the Fall of 2014 Capitol County Children’s Collaborative we will be expanding our services to include children and adolescents with chronic medical conditions along with their mental health challenges. (Behavioral Health Home). The Children’s Behavioral Health Home (BHH) will become a part of the state’s larger Children’s System of Care (CSOC), and will be managed by the Contracted Systems Administrator (CSA), PerformCare. Each BHH will be a designated Care Management Organization (CMO). NJ will enhance the current care management teams to include medical expertise and health/wellness education for purposes of providing fully integrated and coordinated care for children who have chronic medical conditions. Health Home Services are a “bridge” that connects Prevention, Primary Care, and Specialty Care.

9 BHH Continued A Health Home is a person centered medical home that is equipped to fully address the medical, behavioral health, and social supports and service needs of individuals with chronic conditions. Focus on improving self-management and wellness and can be delivered in a variety of models. 16 Education Partnership The mission of the Mercer County CIACC Education Partnership is to promote, develop, and enhance collaborative efforts between school, behavioral health and child protective service systems and other interested parties to improve the well being of children in Mercer County.

17 Education Partnership cont. The Partnership was conceived in 2006 by members of the Ocean County CIACC who recognized a need for ongoing, standardized exchange of procedural information between local schools, the child protective service agency and children's behavioral health programs. The services and supports available for children are continually growing and evolving. Through this Partnership, professionals from each of the three systems are provided up-to- date, ongoing training and education on the services that are available and how to access and effectively coordinate with those services, which will help ensure that children receive the help that they need. Through enhancing the knowledge of and communication between professionals, Mercer County children may see the full benefit of these systems working together to meet their multifaceted needs.
