IMMUNIZATIONS > 28 DAYS E. HEPATITIS OR JAUNDICE > AGE 11 F. FOREIGN TRAVEL (MALARIA COUNTRY) > 12 MO. G. PREGNANCY: > 6 MONTHS AFTER DELIVERY H. TRANSFUSION > 6 MONTHS I. TATTOOS > 1 YEAR
