Patient Case Neurosurgery decided to administer idarucizumab
80 y/o male – presenting to the ED after slipping and falling on ice. Per wife, patient fell backwards hitting his left side and head Hx of atrial fibrillation, diabetes, hypertension, coronary artery disease, hyperlipidemia, stroke, CAD CT head: subdural hematoma Medications prior to arrival Dabigatran 150 mg bid Aspirin 81 mg daily Lisinopril 10 mg daily Amlodipine 10 mg daily Labs upon presentation: Na: 139 mmol/L K: 4.3 mmol/L SCr: 2.5 mg/dL (baseline 1.2 mg/dL) BUN: 57 mg/dL Neurosurgery decided to administer idarucizumab and possibly go to the OR for evacuation of hematoma

Mechanism of Action It is a humanized monoclonal antibody fragment (Fab) that binds specifically to dabigatran and its acylglucuronide metabolites with an affinity for dabigatran that is ~350 times greater than that of thrombin Praxbind®.
