35year old man, In a rural ED after a rollover MVA GCS 6 on arrival now intubated by the local anaesthetist CT brain shows small right subdural haematoma, no midline s hift Chest x-ray shows pulmonary contusions but no pneumothora x Pelvic x-ray is normal Nearest major trauma centre is 4 hours away by road, or 1.5 h ours by air (helicopter) Prepare a major trauma patient for transfer
