8 Personal Health Assessment Advantages Assist with disease management Assist with clinic formulary management Identifies risk factors that can be addressed through education and or medication

9 Personal Health Assessment Participation In February covered employees and dependents had the opportunity to participate in the PHA throughout the District Over 435 employees and dependents took the assessment

10 How will we get maximum participation? Schedule a specific date for maintenance personnel at Support Services Building Schedule additional dates and time at designated secondary campuses Schedule times directly with the clinic To receive the reduced premiums through the open enrollment system in July, PHA must be completed by June 30, 2013

11 How will we get maximum participation? Adjustments will be made manually after open enrollment is complete All PHAs must be completed by September 1, 2013 to receive discounted premiums Employees, Spouses on the medical plan required Dependents over the age of 18 are strongly encouraged to participate in the PHA

12 Annual Participation in PHA Annually PHA will take place in February All assessments must be completed by May 31, 2014 to receive the premium discount for the 2014-2015 plan year.

13 What about New Hires? All new hires must complete the PHA within 90 days of effective date of coverage For example: – Professional employee with an effective date of coverage September 1, 2013 would have until January 1, 2014 to complete the PHA process or be subject to the higher premium
