4. 4 Homeless Outreach Initiative: Overview A three phase approach to increase delivery of health information to people experiencing homelessness.

5. 5 Outreach Activities  Complete a 15-question agency survey  Allow us to administer a 13-question survey to your clients  Provide space and time for the homeless community survey at your agency

6. 6 Benefits  Designation as a DOH-Escambia partner for the Homeless Outreach Initiative  Press coverage for participating in the initiative  Final report that will include findings from the agency and homeless surveys  Clients will receive incentives for completing the survey
