4. Present illness PTA 10years : 작업도중 4 층 높이에서 추락, 하반신 마비 됨 이후 주로 침대에 누워있거나, 휠체어로 이동생활. 09. 1. 13 : 자고 일어나 보니 오른쪽 허벅지가 단단하게 부어 있음을 확인, 왼쪽도 오른쪽 만큼은 아니지만 붓고 단단해짐. 09. 1. 13 : 상기 증상으로 내과에 협진 의뢰 받음.

5. Past history DM : 7~8 년전 진단받고 PO med. HTN/Pul. Tbc/Hepatitis : none Op. history : 2004 년 Lt. Limb 에 DVT 생겨 혈전제거술 (IVC Filter) 시행

20. Plan 더 이상의 새로운 혈전 발생 억제 –IV Heparin 80IU/kg IV 이후 18IU/kg/hr 6 시간마다 aPTT 측정 5~7 일 정도 사용 –Oral Warfarin 일일 1 회, 3~6 개월간 투여 5~6 일간은 헤파린과 병용투여 –Leg elevation – 탄력 스타킹

27. 심부정맥혈전증 DVT Risk Factors –Immobility –Increase the propensity for blood to clot. –Previous DVT / Family history of DVT –Dehydration. –Recent surgery, especially orthopedic surgery. –Age above 40. –Estrogen hormone therapy including OC –Pregnancy. –Previous or current cancer.

31. Treatment Stable –IV heparin + 경구 항응고제 5~7 일 – 이후 경구 항응고제 최소 3 개월 이상 Unstable –Thrombomytic therapy – 이후 stable 시와 동일하게 치료 항응고제 금기시 –IVC filter

32. Treatment Heparin IV – 기전 : Heparin 과 Antithrombin 결합 -> 이결합체는 thrombin 과 Factor Xa 를 억제 – Heparin 은 AT III 와 결합해 AT 의 억제 작용을 더욱 강 화시킴 – 의심 환자 모두에서 금기 아니면 즉시 투여 –aPTT 로 monitoring : control 의 1.5~2.5 배로 유지 – 부작용 : 출혈 (m/c)

35. Oral warfarin – 기전 : Vit K dependent factor II,VII,IX,X 억제

36. Treatment Oral warfarin –5~6 일간은 헤파린과 병용 이유 : 와파린이 실제로 항 응고 효과를 나타내려면 3~4 일 필 요 투여 초기에 Protein C,S 가 먼저 감소하여 오히려 과응고현상 초래 가능 – 일일 1 회 경구투여, 3~6 개월 –PT 로 monitoring : INR 을 2.5 로 유지

37. Treatment IVC filter – 적응증 항응고 요법 / 혈전용해제 금기시 (active bleeding, CVA Hx) 충분한 항응고요법에도 불구하고 PE 재발시 – 국소마취후에 jugular v. 통해 경피적으로 삽입
