1
Using Informatics To Promote Community/Population Health Doris Glenn V. Flores, RN
2
Objectives: Provide overview of community and population health informatics Describe informatics tools for promoting community and population health Define roles of federal, state and local public health agencies in the development of public health informatics
3
What is CHN? is a synthesis of nursing practice and public health practice applied to promoting and preserving the health of populations (ANA).
5
COMMUNITY HEALTH NURSING SYSTEM DEVELOPMENT Management Information Systems (MISs) – is a system which transforms data into information to measure outcomes, track client progress, and exchange healthcare information among healthcare professionals. MISs 4 Domains: -Population interventions and outcomes related to epidemiologic and/or mortality/morbidity trends -Skilled nursing care for individuals and aggregated populations -Specific diagnostic care and/or treatment to special populations in the community -Intermittent, episodic or preventive outpatient care
6
PUBLIC HEALTH Coordinated effort of the local state and federal levels whose mission is fulfilling the society’s interest in assuring conditions so people can be healthy (IOM).
8
FUNCTIONS OF PUBLIC HEALTH -assessment and monitoring of community and population health -formulation of public policies public policies -accessibility and cost effectiveness of care and disease prevention services -evaluation of the effectiveness of care
9
Public Health Informatics Represents a systematic application of information and computer science and technology to public health, research and learning. IT application which focuses on health promotion and disease prevention in population and communities.
10
DEFINING AND GUIDING PRINCIPLES OF PHI Promotion of health Disease and injury prevention Exploration of vulnerable points Reflection of governmental context practice in public health
11
Community Health Risk Assessment (Threat and Risk Assessment) THREAT Harmful act such as deployment of a virus or illegal network penetration RISK Expectation that a threat may succeed and potential damage may occur
12
HEALTH RISK ASSESSMENT Used to estimate whether current or future exposures will pose health risks to broad populations Used to weigh the benefits and costs of various program alternatives for reducing exposure to potential hazards. Constantly developing process
13
4 BASIC STEPS ASCRIBED TO RISK ASSESSMENT 1.Hazard Identification 2.Exposure Assessment 3.Dose Response Assessment 4.Risk Characterization
14
ADDITIONAL RISK ASSESSMENT TOOLS Suicide Prevention Community Assessment Tool Youth Risk Behavior Surveillance System Behavior Risk Factor Surveillance System National Health Nutrition Examination Survey
15
Agency Support Epidemiology and Monitoring Disease Outbreaks through: Availability of Information Technology Comprehensive Surveillance Effort Processing Public Health Data Using a Standardized Vocabulary
16
Applying Knowledge to Health Disaster Planning and Preparation Availability of data Speed of data collection Special data collections Syndromic surveillance
17
Surveillance Data Systems Store information on exposure or trends in adverse health efforts over a specified period of time that can be used by public health officials for planning, evaluation, or implementation of Public Health Interventions
18
INFORMATICS TOOLS: COMMUNICATION AND DISSEMINATION SUPPORT Revolution of IT 2-way Communication in Healthcare PH Information Systems Dissemination of Information IT Solutions
19
Using Feedback to Improve Responses and Promote Readiness Improvement of Community Health Population Health Data RHIO (Regional Health Information Exchanges) / NHIN (National Health Information Network) Public Health Informatics Standardization of Data
