3 Steps to Receive Accommodations Student completes a “Self Identification & Authorization Form” and Student provides documentation of disability to the Office of Disabilities Services (with appointment). When approved, student receives “Confidential Accommodations Request Form” and provides this to all of their instructors.

6 What happens if…answers to common questions A student comes up to me at the end of class and says they get accommodations but do not have the “Confidential Accommodations Form”. Am I required to give them accommodations? The short answer is, no. You can decide to work with the student until he/she has the form and refer them to the Office of Disabilities Services. I know a student in my class is struggling, can I ask them if they have a disability? No. To protect yourself, the student and PHSC, you can suggest speaking to someone in the ODS and making use of the TLC and free tutors. How do I decide which accommodations are appropriate for the student? The student provides ODS staff with documentation used to decide what accommodations are appropriate, therefore, it is not your responsibility to decide. Contact ODS for any suggestions in implementation. Will the accommodations compromise the integrity of my class or academic program? No. Essentially the accommodations are used to “level the playing field” and not to give an unfair advantage. You should grade the students work just as their peers, no need for leniency.

7 What is Assistive Technology? Assistive technology (often abbreviated as AT) is any item, piece of equipment, software or product system that is used to increase, maintain, or improve the functional capabilities of individuals with disabilities. AT can be low tech like communication boards made of cardboard or fuzzy felt. AT can be high tech such as special purpose computers. AT can be hardware such as prosthetics, attachment devices (mounting systems), and positioning devices. AT can be computer hardware, like special switches, keyboards, and pointing devices. AT can be computer software such as screen-readers or communication software. AT can be inclusive or specialized learning materials and curriculum aids. AT can be specialized curricular software. AT can be much more, including electronic devices, wheel chairs
