2 SmartCare – A short description of the SmartCare project SmartCare aims to define a common set of standard functional specifications for an open ICT platform enabling the delivery of integrated care to older European citizens. A total of 23 regions and their key stakeholders will define a comprehensive set of integration building blocks around the challenges of data- sharing, coordination and communication. Nine regions pilot their own integrated health & social services to combat a range of threats to independent living commonly faced by older people. Belit developed innovative solution that seamlessly integrates health and social care, and also allow involvement to informal care providers, as well as third sector. Service: Integrated care to older European citizens Scope: 4.260 users Partners: 42 partners in 23 regions Pilot locations: nine pilot sites in 9 countries Duration: 36 months (3/2013 - 3/2016) New services have been developed by Belit - web portal and mobile application for all participants in the project – health care and social care professionals, informal care providers and care recipients. These services enable permanent monitoring of health status of care recipient, remind care recipient on tasks and activities that he or she has to do during the day, all along providing oral and written communication with informal, as well as with formal care providers. 3 SmartCare – Innovative and flexible tool for integrated care Belit’s SmartCare solution implements innovation which allows unified approach and seamless integration of health and social care processes, as well as integration of informal carers in the process, and involvement of third sector. It flexibility assures that: o additional stakeholders can be incorporated in the process of defining relevant set of data; o input of data from care professionals, informal carers and care recipients; o import of data from various sources; and o monitoring of care recipients. This solution provides, but is not limited to, integrated home support after hospital discharge, as well as integrated long- term care support. SmartCare incorporates innovation towards seamless integration of health an social care processes, and allows for integration of informal care providers in the process, giving an effective tools for easier and more efficient monitoring of care recipients health status and their wellbeing, regardless of their location. 4 SmartCare – Web portal SmartCare web portal, together with mobile application, enables deeper integration of social and health care provision, real time monitoring of patients vital parameters and real-time exchange of information between the different health and social care professionals. SmartCare portal supports the coordination of care workflow across service provider organisations. Access to the portal is also given to service users (care recipients) and, upon consent from care recipients, also to informal care providers. Web portal is a central place for data exchange, review of data and communication. Web portal is closely integrated with mobile application and all its functionalities. SmartCare platform enables real time monitoring of patients vital parameters and real-time exchange of information between the different health and social care professionals. This is a central tool that enables deeper integration of health and social care.

5 SmartCare –Mobile app SmartCare mobile application is a valuable tool for care recipients, as it provides reminders and reports, offer additional communication channel, and significantly facilitates input of data, improves self confidence and encourages independence. Belit developed SmartCare mobile application primarily for care recipients (service users) and informal care providers. Mobile application is a valuable tool for care recipients, as it facilitates data input, alerts on defined tasks and activities, provides easy to use reminders and reports, and also enables additional cannel of communication with, both, formal and informal care providers. Application also grants informal care providers valuable tool to check on care recipients that they care for, and ensure that data is correctly entered and done in a proper manner on time or done at all. In a case where service user is not able to input data herself, informal care provider can do it for her trough his own application. SmartCare mobile application also allows remote update of activities e.g. easy refill of prescriptions or scheduling of appointments, thus saving time and unnecessary travelling.
