Transition Planning and Supports for Persons with Disabilities Planning your child’s education with the future in mind.

What is Transition Planning? Outcome based planning for post secondary goals. Based on the student’s needs and takes into account their preferences, interests. Federal guidelines require by age 16. Texas requires it to begin by at least 14. AISD prepares students for participation at the 5 th or 6 th grade level. REALITY-Starts when your child begins the public school experience.

After we get the information, then what? The creation of Post Secondary Goals Goals are created in areas of interest or those needed. Examples: o After graduating from high school, John will work full time in the field(s) of business ownership, artist, professional sports. o After graduating from high school, Amy will earn a bachelor’s or associates degree from a college or jr. college in the area of business or art. o After graduating from high school, Steven will live on his own. o After graduation from high school, Julie will participate in volunteer opportunities in Arlington.

Once we create the goals, then what happens? Coordinated sets of activities are created promoting progress toward goals. Examples: Research careers in areas of interest to determine job responsibilities and outlook. Research and register for electives offered in areas of interest. Research costs of living in an apartment. Research volunteer opportunities and contact groups for information.

Levels of student participation in the ARD meeting

Elementary 6th graders and some 5th graders are encouraged to participate in ARD meetings. Possibilities -introductions only -staying through review of progress -staying through entire meeting

Jr. High Continue to build self determination and empowerment. Students encouraged to attend ARD meetings. Begin formal transition planning process.

High School Continue to improve skills for self- determination. Continue to improve skills for self- determination. Students encouraged to be active participants in ARD meeting. What can be done right now? Work with your child on developing goals for the future. Identify activities or objectives that can be met to meet your child’s goals. Assist your child in accomplishing those tasks. Monitor your child’s progress in all areas. Review their goals annually. Keep in touch with teachers and counselors. Seek outside supports when needed.
