KJHS DISTRICT 111 SCHOOL BASED HEALTH CENTERS

MISSION STATEMENT The Health Center’s mission is to improve the physical, dental, social and emotional health of students of Kankakee School District 111. Our interdisciplinary services emphasize prevention and early intervention. By reducing barriers to accessing care, we strive to provide these services in a caring and culturally competent environment. We educate students, families, and community groups to encourage healthy lifestyle practices, personal and academic success and reducing morbidity and mortality.

BACKGROUND HISTORY The KJHS Health Center opened in 1987. It was one of the original 5 State funded School based Health Centers in Illinois. Enrollment blossomed, and a second site opened at the high school. The KHS site was originally meant to be a satellite center. That site became busy too, and needed to be staffed full time. Our organization has one of the few grants administered by a school district, not a health organization.

COMMUNITY PARTNERS IDPH provides operational grant funding Orland Park Dental Services and The Kankakee Dental Society provide grants for dental services Kankakee County Health Department Local Title X family planning agency Kankakee Health Center also partners with outside service learners from schools such as : Kankakee Community College Olivet Nazarene University St. Xavier University St. Francis University

PROGRAM OVERVIEW The Health Centers of Kankakee School District 111 are focused on serving the students of this district beginning with children as young as 3 years old (Head Start program) all the way to High School Seniors. Our health centers provide health,dental and mental health services to all our students.

First aid Acute and chronic illness management Physical exams for school and sports Immunizations Health education Nutritional education Onsite dental services three times per month at the KJHS site. Mental Health services Onsite labs such as urine dipstick, pregnancy testing, and strep tests. Chlamydia and gonorrhea screenings and other send- out labs. SERVICES PROVIDED

PATIENT CONSENT AND ELIGIBILITY Our SBHCs utilize a parental consent and medical history form in order to obtain permission to treat the students at our centers. We obtain students’ health insurance and eligibility information via the consent form. Eligibility is verified at the time of visit, always for immunization visits, and whenever else possible.

ENCOUNTER DATA Our Health Centers are currently tracking: Student Enrollments Student/patients with increased BMI Student/patients with asthma

NOTED ACCOMPLISHMENTS The KJHS Health Center has been operational for 28 years. Kankakee School District 111 received two HRSA grants for construction, expansion and equipment. We serve the children in the 13 Schools that comprise the Kankakee School District 111.

PROGRAM GOALS Growth is welcomed at Kankakee School District 111 Health Centers, and we strive for a continuous influx of new enrollees to replace each year’s graduates, which is about 12 percent of our enrollments. The Health Centers would like to have all students in the KJHS and KHS buildings be medically compliant for physicals and immunizations. Improved billing capabilities are also a goal for our Centers, however our main goals at present is for our EMR to be smoothly functioning and linked with MCOs for reimbursement capabilities.

Kankakee School District 111 School Based Health Centers are looking to contract with managed care providers as participating/contracted primary care facilities.
