5 Introduction: Sister Joanna Bruner Family Medicine Center -7,500 patients in our panel -40% are uninsured -We have a high level of cultural and language diversity -We are a low resource clinic with a “Do more with less” ethos

8 What Is The Hook? My journey to improve our clinic has led me to this simple conclusion: I can not do this on my own!!! The model of the “heroic leader” does not apply to our process of transforming into a PCMH. Has anyone else had that experience?
