4 Who is Eligible for the PSU Student Health Insurance Plan?  All Domestic students taking 5 or more in-load credits  All International students (J1 and F1) taking 1 or more credits (dependents are not eligible for the student plan).

5 How Much Does it Cost? 2016-2017 Academic Year: TBD 2015-2016 Academic Year: Fall (9/20/15 – 1/3/16) = $776 Winter (1/4/16 – 3/27/16) = $776 Spring/Summer (3/28/16 – 9/19/16)= $776 Summer only (6/20/16 – 9/19/16) = $590 The insurance is automatically put on your tuition bill when you register for classes.

6 If I Qualify, How Do I Waive Out of the PSU Plan?  Come to SHAC for student waiver assistance during the first two weeks of each term  Hours are Mon-Fri 10am – 4pm  Bring copy of your insurance card  Or waive out online at www.pdx.edu/shac by the waiver deadline www.pdx.edu/shac

7 You will receive a medical ID card in the mail before the term begins. Keep it in your wallet.

8 Where to go for medical help: First, try to come to SHAC. If SHAC is closed and/or it is urgent but not life threatening, use Urgent Care. If life threatening emergency, call 911 or go to nearest hospital.

10 Even if you waive out….. You STILL can come to SHAC for services. Just bring in your insurance card.

11 Center for Student Health and Counseling (SHAC) 1880 SW 6 th Avenue, Suite 200 (on the corner of SW 6 th and Hall, up the street from McDonalds) Eligibility for SHAC services: All students taking 5 or more credit hours pay a Student Health Fee. The Student Health Fee is included in student tuition. This provides you access to SHAC services.

12 SHAC Services Health Services: Primary care with a nurse or a doctor Lab and x-ray facilities Immunizations/Vaccines Screenings for sexually transmitted infections Women’s health exams Contraceptive counseling and prescriptions Free condoms, bandages, cough drops, headache relief, etc. (in the lobby) Rapid HIV testing Referrals to outside specialists Exams and support for sexual assault victims

13 SHAC Services Counseling Services: Brief individual therapy Group therapy Crisis/walk-in services Mind Spa relaxation room Learning disorder and ADHD assessment

14 SHAC Services Dental Services: Full comprehensive exams Fillings Root canals Crowns, partials, and dentures Night guards Custom bleaching trays and sports guards Oral surgery/wisdom teeth extraction Dental emergencies *Please note that Dental Services incur additional fees.

15 SHAC Services Testing Services: Accommodated testing if you need to make up a test or need extra time for a test Secure storage for personal belongings Computer and paper-based testing High security standards Adherence to PSU Code of Conduct

16 SHAC Services Health Promotion: Programs and interventions aimed at influencing and enhancing individual, group and institutional health and safety such as: Tobacco cessation counseling Health coaching Outreach activities targeted at PSU students and the campus community
