Requirements for Applicants: Physical Environment Management/staffing Record Keeping House Rules Residency Requirements

For Instance: √ Entrance and exit must be controlled √ Repair, maintenance, cleanliness, and attractiveness are critical √ Good neighbor policies assure that the home and its residents are accepted as part of the community √ There is a policy for drug testing. √ Sleeping rooms are adequate, private and are not used for any other purposes √ Minimum fire prevention requirements √ Resident records, sign in/out sheets, personal information

House Rules: Mandatory attendance at a weekly house meeting No alcohol, items containing alcohol or illegal drugs Completion of a formal alcohol or drug recovery program, or documented stability in a self-help group.

Ethics are Core to Our Program: 1. Be dedicated to uphold the dignity and worth of all human beings. 2. Provide an alcohol and illicit drug free facility. 3. Operate the SLE within all city, county, state, and federal laws, rules, and regulations. 4. Allow no physical or threatening violence at the SLE. 5. Maintain the SLE consistent with the quality of the neighborhood.

Ethics are Core to Our Program: 6. Assure that no weapons are allowed on the property of the SLE. 7. Assure that neither management nor staff will become personally involved with a client of the SLE. 8. Maintain good relationships with neighbors and community. 9. Respect the privacy and personal rights of all residents. 10. Maintain a clean and safe SLE for persons in recovery.

Complaint Resolution Responding, Resolving.

Complaint Procedure: Complaints come in from consumers, residents, other owners, regulators, elected officials. Investigate by phone, in person, or in writing, or all Counsel owner/manager, follow up to see if solution is reached. Refer to appropriate health, welfare, or law enforcement agency as warranted. Revoke if cooperation is not forthcoming.
