3  The Oklahoma Living Choice Project promotes community living for adults who have disabilities or long-term illnesses. The project gives Oklahomans more options for managing their health care needs and adding more balance to the state’s long-term care system. The OHCA is the lead state agency in the Living Choice Project, which is funded with a grant from the Centers for Medicare & Medicaid Services (CMS) under the Money Follows the Person Rebalancing Demonstration.  Living Choice is similar to the better known ADvantage program (which is a 1915c Home and Community-Based Services waiver) but there are fundamental differences. Whereas ADvantage helps an at-risk population stay at home instead of going into a nursing facility, Living Choice helps individuals move out of institutional settings into their own homes in the community. However, support services and reimbursement rates for these programs are similar. Money Follows the Person (MFP) Living Choice Demonstration Program (LCP)

4 LIVING CHOICE PROGRAM Serves three populations statewide: The physically disabled (ages 19-64) Adults 65 and older The intellectually disabled (DDSD) To be eligible, participants must: Be at least 19 years of age Qualify for SoonerCare (Oklahoma’s Medicaid program) for at least one day prior to transition Have lived in an institutional setting (i.e. nursing home, intermediate care facility) for at least 90 consecutive days prior to transition Voluntarily want to transition back into the community Be willing to play an active role in their plan of care Due to the ADvantage waiver’s age restrictions, Living Choice offers a way for more Oklahomans to achieve independence in their lives. Living Choice also offers a number of additional support services, including the use of transition funds (a one-time $2400 allotment to help participants set up their housing needs).

7 LIVING CHOICE PROGRAM OU PAT PROCESS OU PAT Receives Common Intake Referral Form (Contact Mbr within 1-2 days of receipt of Referral) Initial Visit (Initial Visit w/ Mbr is expected within 3-5 days from first contact) LCP Pre-Assessment Form QOL Survey Participant Consents & Rights Release of Information UCAT I & III Submit all docs completed at Initial Visit to assigned LCP Coordinator *Pre-Assessment activities are to be completed within 30 days of initial visit with member.

9 Quality of Life Survey Time spent administering the QOL Survey should not be added to the total number of units spent on visit QOL Surveys are paid $100 each and therefore will not be paid out of Alternative Funds or Transitional Units A separate invoice can be submitted at the end of the month with all QOL Surveys administered for that month

12 IS THIS MEMBER PROGRAM APPROPRIATE FOR THE LIVING CHOICE PROGRAM? LCP Staff will then send pre-assessment docs to the TC/CM the member selected If member is deemed program appropriate, Care Management will notify LCP staff Care Management will forward to the Dr. for review and/or recommendation in order for member to be program appropriate If Care Management still has questions or concerns in regards to member’s health and safety, an RN visit will be made by Care Management Care Management will review the member’s documents and determined whether they are program appropriate or not LCP Coordinator will forward docs to Care Management *LCP Pre-Screening Form*Consents & Rights*Release of Information*UCAT I & III Care Management

13 TRANSITION VS NON-TRANSITION Transition! If a member successfully transitions back into the community, the LCP Coordinator will contact the OU Pre-Assessment Team with the Prior Authorization number, and number of units approved for which they will be able to bill. Non-Transition If a member unfortunately does not transition into the community, the LCP Coordinator will contact the OU Pre-Assessment Team with the number of units approved so that an Alternative Funds Invoice can be submitted for payment.
