1 CEO Commitment A commitment form will be signed by the CEO committing to release time for appropriate staff and visible support from the hospital’s C-suite for participation in the program. They will further agree to: Name a GAPP contact who will be responsible for assuring achievement of commitments Complete evaluations as requested regarding the overall program and individual activities Use frontline defects analysis to identify, test and refine practical strategies to reduce harm related to specific advanced topic(s) including public reporting of progress for their selected topics; Help to recruit appropriate content experts and staff to implement front line defects analysis to determine a variety of tests of change and create at least one process measure related to each topic that will show improvement in an outcome or surrogate outcome measures Recruit frontline staff to serve as faculty to spread the practices, both locally and nationally; Incorporate an emphasis on patient-centered and family-focused care, beginning prior to admission where plausible, and continuing immediately upon admission

2 CEO Commitment A commitment form will be signed by the CEO committing to release time for appropriate staff and visible support from the hospital’s C-suite for participation in the program. They will further agree to: Use intentional rounding and communication through white boards to facilitate discharge teaching including teach back with both family and patient to promote a clear understanding of condition, medications, follow and community resource to promote smooth transitions and reduction in readmissions Provide hospital PR input to the public information work group Assist GAPP in sharing lessons learned with PfP and other government contractors as appropriate Help measurement work group determine appropriate process, outcome or surrogate outcome metrics and ambitious results targets Participate in the national measurement and accountability system; Participate, when requested by GAPP, in national weekly conversations, webinars, affinity groups, content development and spread based on participating hospital’s specific work For lead hospitals, assist in promulgating duties and functions for staff leads in addition to serving as central LEAPT contact and chair to groups identified above.
