Physician navigates to Sick Leave Attestation form
Sick Leave Attestation request filled in / retrieved by Physician

Submit form to HAAD for processing SL attestation request
Save form in Draft mode for future submission No
HAAD Medical Board Review process
Provide additional details requested by HAAD to support SL attestation request
Print Certificate, stamp & sign and deliver to patient

We expect Physicians (Users) who will submit Sick Leave Attestation requests to register and acquire user ID’s at
HAAD will promptly authorize usernames and passwords to all valid requests from users, such activated user ID’s will receive an confirming activation
