2. 2012 (2012-2013) Annual Performance Report (APR)

APR Overview  The APR is written to report on our state’s performance toward the Indicators outlined in the Colorado State Performance Plan (SPP)  SPP sets targets and outlines improvement activities  APR includes actual data as it compares to targets in SPP and progress on SPP improvement activities  The Office of Special Education Programs (OSEP) requires States to publicly report APR data on local early intervention programs

APR Overview Cont.  The OSEP also requires States to make status determinations for local early intervention programs based on APR compliance data and other information, including timely correction of noncompliance  The OSEP also makes status determinations for States based on performance on APR compliance data, which includes timely correction  Colorado’s 2011-12 status determination was “Meets Requirements”

5. APR Overview Cont.  2012 APR is reporting on the last year of 2005 – 2012 SPP  Beginning work on a new SPP which will include existing Indicators plus a new Indicator 11 State Systemic Improvement Plan

Actual Target Data: 96.04% 2011-12: 98.37% APR FY 2012-13 Indicator 8C: Target 100%

21. Individual Performance Profiles  CCB Individual Performance Profiles were sent out for comment. Comments are due back by December 13 th  Performance Profiles meet the requirement for publicly reporting data for local early intervention programs  Include all of the required measurements of reporting and comparisons between programs and compared to state targets  Status determinations are also publicly reported, although not required

26. Provider Database/Child Outcomes  Contact Connie Fixsen (303-866-4393) or connie.fixsen@state.co.us to transfer a child’s COS record from one CCB to another. The sending CCB should request the transfer. connie.fixsen@state.co.us  If it is known that the child has received services prior to coming to your CCB and has a rating, a transfer must be requested.  Adding the child without first requesting and receiving a transfer creates duplicate records.
