Immunization Requirement Tdap Required for all incoming 7 th graders Protects against Tetanus (Lock Jaw), Pertussis (Whooping Cough), Diphtheria Available from your Primary Care Physician or from Clermont County Health Clinic

Clermont County Health Department 2275 Bauer Rd. Suite 300 Batavia, Ohio 45103 513-735-8400 Call for an appointment to receive the Tdap immunization required before entering the 7th grade. If a parent does not have the funds for the immunization the Health department will waive the fee. You will need to take a current copy of your child's immunization with you. Please call as soon as possible because appointment times fill up quickly.
