2 Working at Height Working at height has been formally classified as a high-risk activity, all workers involved in this activity need to remain up-to-date with industry standards. Working at height refers to any work activity undertaken at any elevated position. Key hazardous events: Person falls from elevated position Person struck by falling objects Working at Heights VideoWorking at Heights Video (3 mins 20 secs) “The way a person falls under 4 metres is the way the person will land”

3 Working at Height – Statistics Australian Work Related Injuries and Fatalities from Working At Height: 232 workers killed in Australia from a working at height fall in 8 years (2003-2011) “5 Mining related fatalities” Half of the falls that resulted in a fatality involved distances of less than three metres. Falls from ladders accounted for the greatest number of fatalities (37 workers died) 6900 workers were hospitalised following a fall from height in the three-year period (2006- 2009) In 2010–11, 7730 claims for serious injury were lodged due to a fall from a height. (equates to 21 a day)

4 Evolution Incidents - Working at Height Contractor was adjusting barricade where there was potential for the contactor to be exposed to open hole without fall arrest protection. Personnel were working without a Permit to Work and Working at Heights Permit. Personnel were also observed exiting an Elevated Work Platform at Height Operator observed working on top of a 4.5m high water tank without any working at heights equipment Contractor working at heights attached to un-approved anchor point, while working under a working at heights permit. Operator working at heights on top of trommel using an incorrect anchoring method and selection No task is so important that it can’t be done safely

5 Evolution Principle Hazard Management The Evolution Principal Hazard Control Manual (PHCM):Evolution Principal Hazard Control Manual Has been developed to define processes and standard requirements applied to areas of significant risk across Company operations. The intent is to prevent injury, fatalities or long term health or environmental impacts associated with major hazards relevant to company activities. “Principal hazards have a potential to result in a fatality and need to be managed effectively across all operations”. 6 Hierarchy of Controls Try to eliminate the risk of working at height Where possible implement above the line controls rather than relying on Administrative controls and PPE

7 Work at Elevation 1. Identify Fall Hazards and Risks 2. Control the Risks 3. Apply Hierarchy of Control 4. Use Fall Prevention Devices/Systems 5. Implement Controls 6. Verify Fall Equipment Inspection and Condition 7. Verify Worker Competencies involved in the work 8. Establish Emergency Procedures/Fall Rescue Plans 9. Communicate with Teams and Check Communications 10. Conduct Work 11. Monitor controls/review changes to work scope or conditions Principle Hazard Management

8 Working at Heights – Dropped Objects Following a number of dropped object events at Jimblebar the contractor workforce organised a field demonstration to their work crew on what the impact of dropped objects could be. Video - Dropped objects demonstration PVC tube was set up on the GTU and different objects were dropped onto a PPE protected water melon to demonstrate the potentially fatal results.

9 Injury Prevention Principles All injuries and incidents are preventable No task is so important that it can’t be done safely Working safely is a condition of employment Management takes accountability for safety performance Everyone is accountable for their own safety and for those around them Everyone is empowered to stop at risk behaviour and control unsafe conditions
